The Form.....

I have a form sitting in front of me. It's a Medical Consent Form for our GP to complete, so Finn can begin a Horse Riding program, its called "Rideability". It's not as big as others, no where as extensive as I have seen before but it has slapped me into reality.......

The following WILL preclude and applicant from riding..............
[] Extremely poor head control
[] Excessive, chronic and/or recurring pain, especially as a result of physical activity
[] Pathological Fractures - i.e. Osteogenesis inperfecta, severe osteoporosis
[] Uncontrolled seizures - categorized by tonic/clonic type occurring more than once a week
[] Acute stage rheumatoid arthritis or juvenile rheumatoid arthritis
[] Open pressure sores, or wounds
[] Unstable spine - including recent spinal injury and/or surgery, atlanto-axial discloation, spondylolysthesis, acute disc herniation
[] Spinal fusion with rod type internal fixation, i.e. harrington or CD rods.
[] Severe behavioral disorders e.g. aggressive episodes resulting in safety concerns
[] Severe fatigue related conditions i.e. multiple sclerosis, muscular dystrophy, ABI
[] Severe clotting related blood disorders i.e. hemophilia and/or Medication/treatments which cause clotting related disorders such as heart conditions/chemotherapy
[] Degeneration/dislocation of the hip joint

The following MAY preclude an aplicant from riding........
[] Poor endurance/fatigue related conditions i.e. MS MD ABI
[] Muscle tone abnormalities - high tone (spasticity) or low tone
[] Osteoporosis (permission required by orthopedic specialist)
[] Scoliosis - especially severe and /or fixed (permission required by orthopedic specialist)
[] Spinal fusion (permission required by orthopedic specialist)
[] Allergies - especially severe anaphylactic reactions
[] Recent Surgery (permission required by surgeon)
[] Acquired Brain Injury in past 2 years (permission required by neurologist)
[] Heart Conditions i.e. postural hypotension, hypertension, coronary artery disease
[] Dysplasia of the hip (permission required by surgeon)
[] High level spinal cord paralysis
[] Medication - drug dosage negatively affecting physical/cognitive function[]
[] Thermoregulatory problems e.g. ABI
[] Hydrocephalus/cranial shunt
[] Respiratory conditions - e.g. Asthma, Chronic Airways Disease
[] Seizures - Type/frequency
[] Gastric feeding tube
[] Skin Disorders - especially circulatory and/or impaired sensation
[] Dizziness/fainting

There is more.....but..... you get the picture

Here's the SLAP...every word that I cannot pronounce, spell or understand has a meaning. It's connected to a child or adult who has a immediate and extended family, health care workers, medical professionals, alternative health practitioners, respite carers, home carers...on...and...on...it goes.

I have the right as we all do, to be BLUE from time to time BUT if you see me "dragging my feet" for more than 24 HOUR'S ...Just say "REMEMBER THE FORM LEONIE" and then I'll regain some perspective!!!

Finn is "all clear" to go horse riding, now we just have to get him on the HORSE!!

Wow what a list! Looks like you need a PHD or at least a medical degree to read through it.

That is great - he has passed and he will be cautious - you know that but then - he won't want to come off!

Yep, I reckon a good solid dose of perspective is one healthy thing. Such mighty fine news that Finn gets to ride!
